Patient Access Representative (Fox Chase)

Coordinate and schedule appointments for new medical and surgical patients using pre-established criteria. Completes new patient registration intake form recording patient demographic and insurance information, referral source as well as key clinical information. Notifies Nurse Navigation of patient intake to ensure follow up for clinical intake and scheduling of patient's visit. Schedule follow up appointments for returning patients via calls from patients and physician orders. Reschedule appointments due to clinic changes and notify patients via phone or mail.
